How do I become a new patient?
First, you will fill out our new patient paperwork that can be found under the forms tab or at our office. Once they are turned in to the office, we will need historical medical records sent from your previous providers. Once we have recieved those, we will work on getting everything reviewed and put on the schedule.
How can I schedule a COVID test?
If you are needing a rapid or PCR COVID Test. You can call our office at (618) 429-9470 to get an appointment. At your appointment time, please pull to the back parking lot behind our building and call us when you get here. A staff member will come out get your test started. If you have not been tested here before, there will be a little paperwork for you to complete. Rapid results are avaliable within 20 minutes. PCR results may take 24-48 business hours.
Affinity isn't my PCP, how can I get on the Testosterone and Weight Loss Management Programs?
If you have not been seen at AFHW but would like to become a member of our speciality services like TRT or Weight Loss. We will need patient paperwork and the required labs to begin the process and get in for an initial appointment. For more information visit the tabs above.
Do you take my insurance?
We accept many different types of insurance plans. Click Here for more detailed explanation of those plans. We also offer, self pay and discounted sliding fees for those people who qualify.
